Building a New Model for Chronic Disease Care

The Logan River Cardiovascular Renal Metabolic Center is part of a growing national movement to improve care for people living with multiple chronic diseases. This center is Utah’s first member of the Cardiometabolic Center Alliance, a collaborative founded by Saint Luke’s Mid America Heart Institute.

About Dr. Clark

Dr. Thomas Clark is a board-certified Diabetologist and a Diplomat of the American College of Diabetology, specializing in chronic disease management for patients with diabetes, heart disease, kidney disease, metabolic syndrome, and obesity.

He completed his undergraduate degree at Utah State University and earned his medical doctorate from the University of Utah. He completed his internship and residency at Naval Hospital Camp Pendleton in Oceanside, California, and received his Diabetic Doctor certification in 2025.

Dr. Clark is the first Utah physician certified by the Cardiometabolic Center Alliance, bringing this new, evidence-based model of care to Northern Utah. He is also fluent in Spanish and Portuguese and happily sees patients needing care in these languages.

About Guideline-Directed Medical Therapy (GDMT)

At the heart of this program is a care approach called Guideline-Directed Medical Therapy (GDMT). That means every treatment plan follows the most up-to-date medical research, ensuring patients receive therapies proven to reduce risk, prevent complications, and improve outcomes.

In this model, Dr. Clark and his team:

Review each patient’s medications and therapies for safety and effectiveness

Coordinate with primary care providers and specialists to keep care consistent,

Focus on prevention and early intervention rather than crisis management.
