The Logan River Cardiovascular Renal Metabolic Center was created to serve patients managing two or more chronic conditions: Most often diabetes paired with heart, vascular, or kidney disease.

These conditions are deeply connected. Diabetes can damage the heart, obesity can lead to diabetes, and kidney disease often follows both.

Under the leadership of Dr. Thomas Clark, Ogden Clinic’s Logan River Cardiometabolic and Renal program integrates cardiology, nephrology, and metabolic care to improve patient outcomes. This collaborative model reduces fragmented care and ensures every patient receives a unified treatment plan designed for their specific health needs.

This program doesn’t replace a patient’s regular doctor, it supports them. Dr. Clark’s team helps patients get on the best treatments for their conditions and keeps close communication with their current providers. The goal is simple: coordinated healthcare, fewer complications, and better long-term health.
